I / We hereby apply for membership of DNOAI – Association of MSOs and confirm that the information furnished in this application and the documents submitted along with it are true and correct to the best of my / our knowledge and belief.

I / We further confirm that I / We are / am operating the business in accordance with applicable laws, regulations, directions and orders of competent authorities.

I / We have read and understood all the governing documents, membership rules and bye-laws of DNOAI, as amended from time to time.

I / We undertake to:

1. Abide by the Memorandum, Articles, Rules, Regulations, Bye-Laws and policies of DNOAI, as applicable.
2. Comply with the decisions of the competent governing body of DNOAI, subject to the applicable rules and laws.
3. Provide information and documents as may reasonably be required by DNOAI for membership, verification, regulatory representation or other legitimate association purposes.
4. Promptly inform DNOAI of any material change in the information furnished in this application.
5. Conduct our business in accordance with applicable laws and regulatory requirements.
6. Maintain the professional integrity and reputation expected of a member of DNOAI.
7. Pay membership fees, subscriptions and other lawful dues within the prescribed time.
8. Understand that admission to membership is subject to approval by the competent authority of DNOAI and does not by itself create any right to representation in any particular matter.
